What to bring to your PCV medical
Your current DVLA D4 form
Bring the correct D4 medical examination report for your application or renewal, with your own sections completed where required.
Your documents
Bring photo ID, proof of address, your current medication list, glasses or contact lenses used for driving, and any requested medical records or supporting clinic letters.
Relevant medical evidence
If you use insulin or medicines that can cause low blood glucose, bring your meter or continuous glucose monitor and at least six weeks of readings.
