What to request

Check your authority before you book.

Medical-record requirements vary between licensing authorities and medical types. Select your medical type and council during booking and follow the records guidance shown there. You are responsible for obtaining the records from your GP surgery and bringing them to the clinic.

Full medical records

When full records are needed, contact your GP surgery early and ask for the full record required for your driver medical. Ask whether the surgery can give you a secure electronic copy or a paper copy to bring. A booking should only be made when you can provide the documents required for that appointment.

Medical summary

Where a summary is accepted, ask your GP surgery for a current medical summary. Bring it in the requested format, together with an up-to-date medication list and relevant specialist letters. The NHS App is not accepted as a replacement for the records required by the licensing authority.

Forms and dates

Bring the current form issued by DVLA, TfL or your local licensing authority. Some authorities set a time limit for records or summaries, so check the booking guidance before requesting them. For HGV, LGV, bus and coach medicals, use the current DVLA D4 form and guidance.

TfL taxi and private-hire applicants

TfL's TPH/204 medical declaration says it should be completed by a registered medical practitioner with access to the applicant's full medical records and includes an in-person examination. Download the current TPH/204 form from TfL and arrange the required records before booking.
