Your DVLA D4 form
Bring the current D4 medical examination report for your application or renewal. Complete your own sections before attending where the form asks you to do so.
Identification and medical information
Bring photo ID, proof of address, your current medication list, glasses or contact lenses used for driving, and any records or supporting clinic letters requested during booking.
If you have diabetes
If you use insulin or medicines that can cause low blood glucose, bring your glucose meter or continuous glucose monitor and at least six weeks of readings.
